Because a terrorist or enemy attack in the current era doesn't have to be a "Dr. Strangelove" nuclear bomb. It can be stochastic terror in the form of a cyber attack on infrastructure, etc.; a couple of drones.
And it doesn't have to be limited to the cities he mentioned (if it were, it would sure seem he was involved in the planning and he was stupid enough to let that slip) - but it's actually an open invitation for stochastic terror that could hit anywhere, any time.
The issue may not be millions of deaths, but it would not be particularly difficult for it to be an issues of hundreds or thousands of injuries or death, or the emergence of danger over hours, days or weeks via cyber-paralyzing an unsuspecting area.
Of course, San Diego would be an attractive target because we are so filled with military bases, and if this city (or County - Camp Pendleton is not in San Diego City, but in San Diego County) is paralyzed - even if few die, the military could be seriously disrupted.
Why ever he said it (if he even knows, or if it was what just came to his feeble mind at the moment) - it only increases danger everywhere and to everyone, to an unpredictable extent and via unpredictable actions.

My point is a long term one. Got relatives and neighbors who still magaize. Their desperation is apparent-quoting falsehoods, blaming Biden, and stretching to irrelevant things (statements from 15 yrs ago, mostly whatsboutisms).
I wonder (and fear) what is going to happen to them when Trump is gone (dead most likely and out of office for sure). They are being confronted with facts and being affected by trumps policies but they still dismiss as fake news or not true.
